I'd start by checking the connection map and verifying it lists
the source dispinterface.

Have you listed IConnectionPointContainer in your interface map?

I assume you are referring to the following, which is indeed in the
map...

BEGIN_COM_MAP(CCoCustomThirdParty)
...
COM_INTERFACE_ENTRY(IConnectionPointContainer)
END_COM_MAP()

In this case, I'd put breakpoints into
IConnectionPointContainer::FindConnectionPoint and
IConnectionPoint::Advise, and try to figure out why the client fails to
advise its sink.
