Thanks; It doesn't happen often, so I can understand why you can't reproduce
it. My goal was more to find out how that association is stored and if
there's some way we could fix it if and when this happens again.

I performed a test with a dialog based MFC application, but unfortunately

could not reproduce all of your issues. I found that the class header and
source file fields are indeed empty and grayed out, however this seems by
design since I tried this on several machines and they had the same
behavior. The difference from yours was that when I clicked OK, the DDX
statement was automatically added to the dialog class source file:
void CADOTestDlg::DoDataExchange(CDataExchange* pDX)
{
CDialog::DoDataExchange(pDX);
DDX_Control(pDX, IDOK, m_btnOK);
}

For your issue, have you checked if this issue occur on other machines?
Also, I recommend that you use Process Monitor to monitor the devenv.exe
to
check if there are some clues in the logs.
